All we need is some simple information to start your Pension Review. You can either contact us and we will collect the required information from you, or just follow these steps:

- Print, complete and sign the Form
- Either post the form back to us at: Challenge House, Sherwood Drive, Bletchley, Milton Keynes MK3 6DP
To avoid delays, do remember to:
- Sign the form (!)
- List all your pensions
- Include all your pension policy numbers (which are essential to start the pension review service
The policy numbers can be found on your annual pension statements or on any communication from your existing provider.
If you have lost touch with your pension provider there is a Government pensions service that traces pension policies, this can either be used on-line at https://www2.dwp.gov.uk/tps-directgov/en/contact-tps/pension-tracing-form.asp or by ringing the Pension Tracing Service on 0845 6002 537.
You can include up to 4 pensions on each form, but if your partner has any pensions he or she will need to complete and sign a separate form. The pension review company will consider all your existing pensions and if appropriate consolidate them into one easier to manage scheme.
